If you cannot open the Application Form you may need to download Adobe Reader from here.
We can also post you a copy of our Application Form along with our leaflet, VAT Relief Eligibility Declaration Form and a FREEPOST envelope that you can use to return any items we require back to us. Note, setting up your account this way takes the longest.
